Anne Madden, PT, DPT Founder & Physical Therapist

Anne founded Proximity Telehealth in 2019 to bring high-quality teletherapy to Idaho schools struggling to recruit related-service providers locally. Today she leads a team of 25+ licensed clinicians serving districts across Idaho — and Proximity has delivered more than 120,000 sessions to Idaho students.

Anne holds a Doctor of Physical Therapy degree from Arcadia University and was an early adopter of school-based teletherapy in 2016, making her one of the most experienced telehealth PTs in the country.

A military spouse and mother of three, Anne lives in Boise with her family.

Trish Graham, M. Ed., CCC-SLP

Operations Manager

Trish leads our clinical team as Manager, drawing on more than 30 years of experience as a Speech-Language Pathologist. She oversees provider quality, supports new team members through onboarding, and works directly with school partners to make sure every student gets the most out of their sessions.

Trish has spent her career building meaningful relationships with students and the schools that serve them — and that's what she's brought to the Proximity team since 2021.

A passionate Carolina Hurricanes fan ("Go Canes"), Trish lives in North Carolina with her family.
